      Incident report (parking damage, fixed object collision)

      After hours call script

      • Ask if anyone involved is injured
      • If incident involves another vehicle, find out if police has been notified and if not advise driver to do so
      • Collect driver name, truck number, trailer number and location
      • Advise driver to take and send pictures of all vehicles/fixed objects involved in the incident as well as record the other driver’s insurance and contact information if applicable
      • Ask driver if truck is operable or needs to be towed
      • Determine whether shipment will be delayed and if so notify broker
      • Advise driver we will be calling them back shortly with instructions
