What is Our Accident Procedure?

Here is what to do in the event of an accident.

All drivers must know what to do – and sometimes what NOT to do – after an accident occurs.

Follow these procedures in the event of an accident:
  • Stop!
  • For Emergency Call 911.
  • Turn off the ignition and set the brakes.
  • Turn on the four-way flashers
  • Look for hazards such as leaking fuel.
  • Note any injured parties. Assist them only if you are appropriately trained. DO NOT move an injured person unless there is imminent danger to that person (such as a vehicle fire).
  • Have a bystander call for help if injuries are involved.
  • If possible, do not move your vehicle until instructed to do so by law enforcement.
  • Position the warning triangles.
    Please take pictures of the scene as it is before moving any vehicles. Include:
  • Pictures of all vehicles involved in the accident.
  • Damages to all vehicles (interior and exterior).
  • Position of vehicles (from every angle of the accident scene).
  •  Any factors that may have contributed to the accident.
  • License plates of vehicles involved, as well as those of any witnesses.
    Document the following information:
  • Names, addresses, and phone numbers for all drivers involved in the accident. If a commercial vehicle is involved, get the name and address of the company and identifying information such as license plate numbers, unit numbers, DOT Number and/or VIN numbers.
  • Names, addresses, and phone numbers for any other involved parties.
  • Exchange insurance information with appropriate parties.
  • If you have struck an unattended vehicle, leave a detailed note in a secure place with your name, company name, address, and phone number.
  • Write down the names of witnesses and their license plate numbers, if applicable. Ask them to complete a witness card (found in the truck’s accident kit).
  • Write down a detailed account, including a sketch, of what happened. Use the Accident Report Form.
  • Note weather and road conditions.
  • Cooperate fully with law enforcement and emergency personnel.
  • Be polite to everyone at the accident scene.

    DO NOT:
  • Discuss who is at fault.
  • Get into an argument.
  • Sign anything.
  • Speak with the media or others at the scene, with the exception of law enforcement personnel or representatives of Logiflex Inc. and/or its insurance carrier.


Any accident must be reported immediately to the Safety Department of Logiflex Inc regardless of how minor it may appear.

Post-accident drug and alcohol testing
If a driver is involved in a DOT recordable accident (caused bodily injury requiring medical attention immediately away from the scene, and/or one vehicle must be towed due to disabling damage) and the driver receives a citation for a moving motor vehicle violation, or is involved in a fatal accident, that driver is required to be both alcohol and controlled substance tested.
